Fee
My fee for sessions is $200 for individual and $250 for couples. Documentation will be provided for you to submit to your insurance for any potential reimbursement you may be eligible for.
Insurance
I am an out-of-network provider. If you have out-of-network coverage, you can submit superbills for reimbursement. To know what to expect financially, the following questions are helpful to ask your insurance provider:

Do I have out of network mental health coverage? If so, at what rate will I be reimbursed for CPT code 90834 (psychotherapy, 45min) and CPT code 90837 (psychotherapy, 60mins)?
Do I have a deductible I must first meet? If so, how much is that?
What are the UCR (“usual, customary and reasonable”) charges you allow for these services – CPT code 90834 (psychotherapy, 45min) and CPT code 90837 (psychotherapy, 60mins)?
Is there a maximum amount you will pay for an office visit?
Do you cap the number of visits in a year?
Is there a specific claim form I must use to submit claims and if so, where can I access that?
How much time after a visit do I have to submit a claim?
